9. Glaucoma
10. Parkinson's Disease
11. Post-Traumatic Stress Disorder
12. Multiple Sclerosis
13. Intractable Pain
14. Alzheimer's
15. ALS
16. Traumatic Brain Injury
17. Consussion
18. Chronic Pain associated with Fibromyalgia or Sickle Cell Disease
19. Conditions resulting in the patient receiving hospice care or palliative care.
